Promising late-breaking results from the Phase 2 IMGN853-0420 trial (NCT05456685) were presented at the annual meeting of the Society of Gynecologic Oncology (SGO), held April 10 – 13, 2026, in San Juan, Puerto Rico.
The study highlights the efficacy and safety of mirvetuximab soravtansine (Elahere®; AbbVie) plus carboplatin, followed by maintenance monotherapy with mirvetuximab soravtansine, in patients with recurrent, platinum-sensitive ovarian cancer (PSOC) expressing folate receptor alpha (FRα).*
Mirvetuximab soravtansine is a first-in-class ADC targeting FRα that delivers a potent DM4 payload directly to cancer cells while sparing normal tissue. It is approved for platinum-resistant, FRα-positive ovarian cancer and is being actively evaluated for platinum-sensitive disease.
In this study, nearly half of the participating patients had prior exposure to a polymerase inhibitor (PARPi), a patient population that may experience reduced responses to subsequent platinum-based chemotherapy [1][2][3]
Key Study Highlights
- The trial enrolled 125 patients with measurable, recurrent PSOC and FRα expression (≥25%), all of whom had received one prior platinum-based chemotherapy regimen.[2]
- Objective response rate (ORR) after combination therapy was 62.7% in the FRα ≥50% subgroup and 62.4% in the overall population.[2]
- 81% of patients continued onto mirvetuximab soravtansine monotherapy, with an ORR of 68% during this maintenance phase.[2]
- Median duration of response was 11.2 months.[2]
- Although nearly half the patients had prior exposure to PARP inhibitors, ORR remained high at 63.9% in this subgroup.[1][2]
Safety and Tolerability
The combination demonstrated a manageable safety profile, with most adverse events being low-grade and reversible, and with reversible ocular changes. The most common grade 3 or higher adverse events included neutropenia (15%), blurred vision (10%), thrombocytopenia (10%), cataract (6%), dry eye, diarrhea, and peripheral sensory neuropathy (all at 5%). Notably, pulmonary toxicity (pneumonitis) was seen in 15% of patients treated for extended periods, mostly mild and reversible, warranting close monitoring during prolonged therapy.
Expert Perspectives and Clinical Implications
This is the first larger study to evaluate a folate receptor-targeting antibody-drug conjugate (ADC) in combination with a platinum agent in platinum-sensitive ovarian cancer and to continue the ADC as maintenance therapy.
“The combination of mirvetuximab soravtansine-gynx and carboplatin delivered strong responses in this Phase 2 study, and many patients continued to experience responses during the monotherapy treatment phase,” explained Gottfried E. Konecny, M.D., Professor of Medicine, David Geffen School of Medicine at UCLA, and the primary investigator of the study.
Konecny emphasized that these findings support further exploration of the combination and maintenance strategy, especially for patients who have previously been treated with PARP inhibitors.
Looking Ahead
While not yet approved for platinum-sensitive disease, ongoing studies are evaluating this combination and maintenance approach in earlier lines of therapy, including the neoadjuvant setting. These results set the stage for additional research, aiming to move ELAHERE® into earlier treatment and improve long-term outcomes for ovarian cancer patients.
